Health Guarantee and Spay and Neuter Contract for puppies adopted from Goldendoodles by Honeydoodles
I, Amanda Smith, breeder, do hereby give the following health guarantee for my puppies subject to the following conditions:
It is the responsibility of the buyer to have his/her new puppy examined by a Licensed Veterinarian within 48 hours of removing the puppy from our property. If the Veterinarian finds your puppy to be sick or in poor health then: the buyer is to contact the seller immediately with that information and arrange to return the puppy to the seller. Upon return, the buyer must provide the Veterinarian’s report showing that the puppy was sold ill or in poor health to receive a full refund or a replacement puppy.
The puppy is guaranteed by the Breeder for 2 years from their date of birth against fatal and life altering congenital, and genetic disease which adversely affects the health of the animal. This includes severe hip dysplasia, kidney, heart, pancreas, spine and liver problems of a life threatening nature. This guarantee does not include, entropies, umbilical hernias, or cryptorchid males. If the puppy is euthanized without the Breeder’s consent, this contract is null and void and no replacement puppy will be given.
If the dog dies within this time frame, the purchaser is to pay to have a necropsy performed to ascertain reason for death. All documents must be provided to the Breeder’s Veterinarian for review. If the reason for death is genetic or congenital, the Breeder will replace the puppy or give a refund of the purchase price. The Breeder must be notified within 48 hours of the Licensed Veterinarian’s necropsy determination and have two reports from two different Veterinarians stating the same problem. The Breeder will replace the puppy with a puppy of equivalent value as soon as one is available and upon surrender of the original puppy. The purchaser is responsible for all shipping or travel costs for a replacement puppy.
The Breeder will not be responsible for the Veterinarian’s fees. The purchaser is aware that owning a puppy means trips to a Veterinarian and understands that he/she is responsible for all costs.
The Purchaser will keep the puppy current on all vaccinations, deworming, flea and tick protection and heartworm prevention. The Purchaser will feed a premium dog food and not allow the puppy to become obese.
The Purchaser is to have the puppy spayed or neutered between 9 and 16 months of age. The Purchaser is to mail a copy of the Veterinary services to the Breeder verifying that the surgery has been done. If the puppy has not been altered by 16 months of age, the 2 year health guarantee will be void unless you have purchased breeding rights from the Breeder. If breeding occurs without having purchased breeding rights and signing a breeding contract, the Purchaser is subject to legal action and any fees related to the matter.
If at any time in your puppy’s life you are unable to keep him/her for any reason such as, housing, your pet doesn’t get along with the puppy, etc., the Breeder will take back the puppy/dog. No compensation will be made to the Purchaser. All efforts will be made to find a suitable home for the puppy/dog by the Breeder.
I, the purchaser of a puppy from Goldendoodles by Honeydoodles, hereby acknowledge that I have read, understand, agree and accept the terms and conditions of the guarantee for congenital or hereditary disorders listed in this document. I also understand and agree to the spay and neuter policy listed in this document.
